Page 10 of 11 Third the CDQ needs to be validated in other patients i.e. those outside of celiac societies and self-help groups and those of other ethnical and socio-economic background. Fourth the CDQ is only valid for adults. In conclusion, we suggest that the CDQ represents a potentially important addition to generic measures such as the SF-36 in HRQOL-research in CD. The instrument can be used as an outcome measure in prospective clinical studies, for example of structured education of CD-patients and their families or of gluten-free diet after first diagnosis of CD. Furthermore the CDQ enables comparisons of the HRQOL of European and American celiacs thus giving the opportunity to test if the cross-cultural differences in HRQOL of celiacs revealed by the SF-36 could be replicated by a disease-specific instrument. We hope that its use in clinical and research settings with necessary validations of our proposed translation into the English and other languages will confirm the subscale construction and the psychometric properties of the German version and extend its applicability on patients outside self-help organisations and outside Europe.
